Please be advised that for obtaining original or copies of documents (including birth, divorce, death and education certificates or records) on file in Ukraine you should submit 3 completed copies of the attached questionnaire to the Consulate General of Ukraine. A separate questionnaire (PDFto be filled out for each document. The request will be accepted only if the information provided in the questionnaire is supplied in full and exact detail in Ukrainian . The questionnaire is to be notarized. The fee is $ 10.00 CDN per submittimg of the document search application and $ 35 CAD per issuance of the searched document.

Therefore an applicant requesting a document should enclose a certified check or money order for $ 10 CAD fee payable to the Consulate General of Ukraine. It must be paid in advance and should accompany the search request.

This fee is not refunded if the document or record is not found.
